Monday, January 19th - Girls 10am-12pm | Boys 12pm-2pms (Arrive 30 mins prior to start time)
Tuesday - Friday, January 20th - 23rd, Boys and Girls 3:30pm-5:30pm
Saturday, January 24th, Boys by invitation only, Time TBD

Step 1. 2026 Registration for Tryouts
All students interested in trying out for the soccer teams at LHS (Boys, Girls, Freshman, JV, and Varsity) are required to go the following google forms link and register their player information on this 2026 Lakeside Vikings Soccer Tryout Form.
Step 2. Sports Physical Uploaded into Final Forms Account
LHS has partnered with Final Forms to automate the management of required athletic health forms. Go to https://www.finalforms.com/ to setup your account. In the Final Forms site, you will be able to upload your completed physical and electronically sign the additional required forms.
Required forms: need to be submitted on Final Forms before tryouts or ANY practice with the team
Pre-participation sports physical - (click here for blank copy to take to physician - en español haga clic aquí). NOTE: Provide a current physical performed within the last year (performed on January 3rd 2025 or later). Only the last page with doctor signature/stamp and date is necessary to upload (not all 4 pages).
Our sponsor, Atlanta Urgent Care (2738 Clairmont Rd, Atlanta, GA 30329) provides $40 walk-in Physicals (no appointments necessary).
